Clinical Pharmacist

About the role

We are seeking a Clinical Pharmacist to join our hospital pharmacy team. The role focuses on optimizing patient medication therapy through comprehensive inpatient medication reviews, drug‑interaction screening, and antimicrobial stewardship initiatives. The successful candidate will work closely with physicians, nurses and other healthcare professionals to ensure safe and effective medication use.

Key responsibilities

• Conduct thorough inpatient medication reviews to assess appropriateness, dosing, and safety.
• Perform drug‑interaction screening and provide recommendations to mitigate adverse effects.
• Lead and support antimicrobial stewardship programs, including guideline development and audit of antibiotic use.
• Collaborate with multidisciplinary teams to develop individualized patient care plans.
• Maintain accurate pharmacy records and contribute to continuous quality improvement.

Required profile

• Pharm.D or M.Pharm (Pharmacology) degree.
• Minimum of 2 years of experience in a hospital pharmacy setting.
• Strong knowledge of clinical pharmacology, medication safety, and antimicrobial stewardship principles.
