Nurse Practitioner PRN – In‑Home Health Assessments

About the role

Matrix Medical Network is seeking experienced Nurse Practitioners to conduct comprehensive in‑home health assessments on an as‑needed (PRN) basis. You will visit members in their residences or skilled nursing facilities, reviewing medical history, medication adherence, and social environment to identify care gaps and improve health outcomes.

Key responsibilities

• Perform adult and geriatric health assessments lasting up to one hour, documenting medical history, diagnoses, and treatment plans.
• Order and administer point‑of‑care and laboratory tests as required.
• Provide health education, physician referrals, and case‑management referrals according to Matrix protocols.
• Collaborate with primary care physicians, case managers, social workers, family members, and other healthcare professionals.
• Maintain clear, compliant documentation for each visit.

Required profile

• Current, unrestricted Nurse Practitioner license in the state of practice.
• Experience conducting adult and geriatric assessments, preferably in home‑based or community settings.
• Strong knowledge of medication management, chronic disease identification, and care coordination.
• Ability to work independently, manage schedule flexibility, and travel to member locations.

• Performance‑based compensation ranging from $90‑$115 per assessment, with monthly earnings of $1,800‑$11,500 depending on volume.
