Nurse Practitioner PRN – In-Home Health Assessments

About the role

Matrix Medical Network is seeking experienced Nurse Practitioners on a PRN basis to conduct comprehensive in‑home health assessments for adult and geriatric members. Visits last up to one hour and focus on medical history, medication adherence, social environment, and overall health status.

Key responsibilities

• Perform adult and geriatric health assessments, including medical history, diagnosis, treatment planning, and health education.
• Order and administer point‑of‑care and laboratory tests as needed.
• Document findings clearly according to Matrix guidelines and protocols.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Provide follow‑up referrals to physicians, case managers, or ancillary services.
• Deliver services at the member’s residence or skilled nursing facility.

Required profile

• Current, unrestricted Nurse Practitioner license in the state of practice.
• Experience conducting adult and geriatric assessments in a clinical or home‑health setting.
• Strong knowledge of medication management, chronic disease monitoring, and care coordination.
• Ability to work independently, manage time effectively, and adapt to varied home environments.

Required skills

• Proficiency in point‑of‑care testing and basic laboratory ordering.
• Familiarity with electronic health record documentation.
• Effective communication with patients, families, and multidisciplinary teams.
