Specialist Pharmacist – Hospital at Home

About the role

The NHS Borders Hospital at Home team is seeking a Specialist Pharmacist to lead a high‑quality, safe, and person‑centred clinical pharmacy service. You will enable patients to receive hospital‑level care in their own homes while ensuring safe, effective, and cost‑effective use of medicines across settings.

Key responsibilities

• Provide specialist pharmaceutical advice and medicines optimisation for Hospital at Home patients.
• Conduct pharmacist‑led medication reviews and patient consultations (in‑person, telephone, or video), including polypharmacy reviews.
• Practice as an independent prescriber (or undertake prescribing training) to manage a defined caseload.
• Collaborate with the multidisciplinary team to improve medicines safety, support safe transfer of care, and reduce avoidable admissions.
• Contribute to service development, policies, pathways, and governance activities such as audit, incident learning, and antimicrobial stewardship.
• Support education, training, and supervision of colleagues and learners within the wider pharmacy service.

Required profile

• Registered pharmacist with experience in clinical pharmacy.
• Ability to perform medication reviews and manage polypharmacy.
• Willingness to act as an independent prescriber or undertake the necessary training.
• Strong collaborative skills to work within a multidisciplinary team.
