Remote Clinical Pharmacist – Short‑Term Contract

About the role

We are seeking a qualified Clinical Pharmacist to join our remote team on a short‑term contract (2‑3 months). The role involves 40 hours per week, split between a fixed 4‑hour PST window and 4 flexible hours, supporting pharmacotherapy and patient safety initiatives.

Key responsibilities

• Conduct comprehensive drug regimen reviews and therapeutic drug monitoring for diverse patient populations.
• Perform renal and hepatic dosing adjustments and screen for drug‑drug and drug‑food interactions.
• Generate clinical deliverables such as dosing recommendation plans, medication reconciliation summaries, and patient counseling notes.
• Analyze complex patient charts to identify and resolve drug‑related problems (DRPs).
• Collaborate with healthcare providers to ensure safe and effective medication use.

Required profile

• 3–8 years of professional pharmacy practice in retail, hospital, or clinical settings.
• Doctor of Pharmacy (PharmD) or equivalent qualification with an active licensure.
• Proven track record of producing clinical deliverables and strong analytical thinking.
• Ability to work independently in a fully remote environment.
