Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

Our client is seeking a Remote Patient Care Coordinator – also called a Cardiology Patient Navigator – to support a busy US‑based cardiology team. The role focuses on patient communication, scheduling, care coordination and clinical documentation for a high‑volume caseload of 150+ patients.

Key responsibilities

• Manage patient scheduling, reminders and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic or remote outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments and remote patient monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care and escalate clinical concerns within scope.
• Maintain accurate clinical documentation and ensure HIPAA‑compliant records.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN or equivalent).
• At least 3 years of experience in healthcare, telehealth or medical office settings.
• Experience in care coordination or management, preferably in a remote or telephonic environment.
• Ability to handle a high‑volume, remote workload with strong organization and communication skills.

Required skills

• Proficiency with EMR/EHR systems.
• Familiarity with remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
