Remote Cardiology Patient Navigator – Patient Care Coordinator

About the role

Our client is seeking a Remote Patient Care Coordinator (Cardiology Patient Navigator) to support a busy US‑based cardiology team. The position is fully remote, works US Central Time business hours, and focuses on patient communication, scheduling, care coordination, and clinical documentation for a high‑volume caseload.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, remote monitoring workflows, and ensure continuity of care for 150+ patients.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA compliance.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• 3+ years experience in healthcare, telehealth, or medical office settings.
• Proven experience in care coordination or management, preferably in a remote/telephonic environment.
• Ability to handle a high‑volume, fast‑paced workload with strong organization and communication skills.

Required skills

• Experience using remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
