Clinical Pharmacist – Patient‑Centred Care

About the role

The Clinical Pharmacist will join RheumaCARE’s multidisciplinary team in Kollam, delivering patient‑centred pharmaceutical care to individuals with rheumatic and musculoskeletal disorders. Working on‑site, you will collaborate closely with rheumatologists, orthopaedicians and other specialists to optimise medication therapy and improve health outcomes.

Key responsibilities

• Perform medication reconciliation and comprehensive therapy reviews for patients receiving biologics, immunosuppressants and other high‑risk drugs.
• Provide counselling, education and adherence support to patients and caregivers.
• Review prescriptions for accuracy, appropriate dosing, drug‑drug interactions and safety.
• Assist the clinical team in developing and updating medication‑use policies, protocols and patient‑education materials.
• Monitor treatment outcomes, report adverse drug reactions and contribute to quality‑improvement and research initiatives.
• Oversee inventory management of specialised medicines, including compounding and handling of high‑risk products.

Required profile

• Bachelor’s or Doctor of Pharmacy (B.Pharm/Pharm.D) from an accredited institution; postgraduate training or certification is a plus.
• Prior clinical experience in hospital, ambulatory or specialty care, preferably in rheumatology, immunology or internal medicine.
• Strong ability to interpret clinical information and apply evidence‑based pharmacotherapy.
• Excellent communication skills, attention to detail and commitment to patient safety.
