About the role

The Yakima Valley Farm Workers Clinic is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join its Lancaster Family Health Center in Salem, Oregon. You will provide comprehensive, patient‑centered primary care to a largely migrant and Latino community, working within a team‑based medical home model.

Key responsibilities

• Manage a panel of patients, delivering preventive, acute, and chronic disease care.
• Provide culturally competent care, including translation support for Spanish‑speaking patients.
• Utilize the EPIC electronic medical record system for documentation and care coordination.
• Collaborate with physicians, medical assistants, and community health workers to address social determinants of health.
• Participate in community outreach and health education initiatives.

Required profile

• Current licensure as a Nurse Practitioner or Physician Assistant.
• Commitment to serving underserved and migrant populations.
• Ability to work flexible clinic hours, including evenings and Saturdays.
• New graduates are encouraged to apply if they have relevant clinical rotations.

Required skills

• Proficiency with the EPIC EMR system.

• Base salary $127,000–$139,000 per year plus productivity incentives.
