Advance Practice Clinician – Nurse Practitioner or PA

About the role

We are seeking an Advance Practice Clinician – either a Nurse Practitioner or Physician Assistant – to join our Lancaster Family Health Center in Salem, Oregon. As part of a federally qualified health center, you will provide comprehensive, patient‑centered primary care to a largely migrant and Latino community.

Key responsibilities

• Manage a panel of patients, delivering preventive, acute, and chronic care.
• Provide culturally competent care, including translation assistance for Spanish‑speaking patients.
• Collaborate with a multidisciplinary team in a team‑based medical home model.
• Utilize the EPIC electronic medical record system for documentation and care coordination.
• Participate in community outreach and health education initiatives.

Required profile

• Current or recent graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Commitment to serving underserved and migrant populations.
• Ability to work full‑time (0.80 FTE, 32 hours/week) with flexible clinic hours.
• Certified bilingual (Spanish) preferred.

Required skills

• Proficiency with EPIC EMR.

• Base salary $112,000‑$124,000 per year plus productivity and patient‑satisfaction incentives.
