Physical Therapist Assistant – Post‑Acute Care

About the role

We are seeking a dedicated Physical Therapist Assistant to join our post‑acute care team. The successful candidate will work closely with a supervising Physical Therapist to deliver high‑quality therapy services to patients in residential, assisted‑living, or skilled‑nursing environments.

Key responsibilities

• Provide therapy services as outlined in the patient’s Plan of Care, under the supervision of a Physical Therapist.
• Consult with the PT regarding any changes in patient condition or treatment plan in a timely manner.
• Complete accurate documentation, including daily notes, progress notes, discharge summaries, and monthly reports.
• Submit billing, payroll, and expense documentation promptly.
• Communicate effectively with patients, families, referring physicians, and the interdisciplinary team.
• Participate in care‑planning meetings, rehabilitation meetings, and family training sessions.
• Maintain equipment in good working order, report any damage, and ensure a clean treatment environment.
• Attend required meetings, in‑services, and continuing‑education seminars to stay current with clinical practices.
• Adhere to organizational policies, state and federal guidelines, and safety protocols.

Required profile

• Associate of Arts degree in Physical Therapist Assistant or equivalent.
• Licensure or certification as required by state guidelines.
• Current CPR certification.
• Preferably 1 year of PTA experience; 1‑2 years in a residential, assisted‑living, or skilled‑nursing facility is a plus.
• Recent graduates of an accredited PTA program will also be considered.
