Perinatal Health Nurse – Home Visiting Program

About the role
The Georgia Department of Public Health seeks dedicated Perinatal Health Nurses to provide home‑visiting services for high‑risk pregnant, postpartum and inter‑conceptual women in Bryan, Camden, and Long Counties. You will deliver clinical support, case management, and education directly in clients' homes.
Key responsibilities
- Conduct home visits for medically diagnosed high‑risk pregnant and postpartum women.
- Provide nurse case management, referrals, education, and connection to community resources.
- Offer general clinical support and monitor maternal and infant health in the home setting.
- Communicate client status updates to health‑care providers throughout the program.
- Participate in emergency response duties as an essential employee when required.
Required profile
- Associate degree in nursing from an accredited institution.
- Current Georgia Registered Professional Nurse (RN) license.
- Completion of a Public Health Nursing (PHN) course within one year of hire.
- Current CPR certification.
- Valid driver’s license.
