Clinical Pharmacist

About the role

We are seeking a qualified and detail‑oriented Clinical Pharmacist to join our healthcare team. In this role you will ensure safe and effective medication use by reviewing prescriptions, counseling patients, and monitoring drug therapy.

Key responsibilities

• Review and validate prescriptions for accuracy, appropriateness, and compliance with regulations.
• Provide patients with clear information on medication dosage, timing, and precautions.
• Monitor medication therapy, identify potential drug interactions or adverse effects, and intervene as needed.
• Collaborate with physicians, nurses, and other healthcare professionals on medication‑related concerns.
• Maintain accurate patient medication records and documentation.
• Support medication reconciliation and follow‑up activities.
• Stay up‑to‑date with new medicines, therapies, and clinical guidelines.

Required profile

• B.Pharm, M.Pharm or Pharm.D from a recognized institution.
• Valid pharmacy registration/license as applicable.
• Clinical or hospital pharmacy experience preferred.
• Strong communication and patient‑counselling abilities.
• Good documentation and computer proficiency.
• Ability to work effectively with multidisciplinary healthcare teams.
